- Migrator creates _migrations table, tracks executed files by name
- Skips already-run migrations on subsequent deploys
- Schema SQL uses IF NOT EXISTS, DO/EXCEPTION blocks, DROP POLICY IF EXISTS
- Runs before server starts, exits on failure
